What actually is PRF biofiller
PRF stands for platelet-rich fibrin. It’s your own blood spun at a low speed, without anticoagulant, so that platelets and fibrin stay in a natural clotting form. That fibrin acts like a slow-release sponge for growth factors that help repair tissue and stimulate collagen.
When we talk about biofiller, we’re not talking about the same thing. Biofiller is made from the albumin-rich plasma part of your blood, heated to a controlled temperature. The heat denatures albumin, thickens it into a gel, and gives temporary structure. Think of it as a soft, short-lived scaffold.
Some systems brand it as EZ-Gel (patented US System) or i-PRF Gel. The principle is the same: heat the plasma, form a gel, and optionally mix it with PRF to add regenerative potential.
PRF itself doesn’t hold shape. Biofiller gives that shape, but both rely on your own proteins.

How long it lasts (the honest version)
If you’re seeing your results fade by 6-8 weeks, you’re not doing anything wrong. That’s the biology. The gel itself is absorbed quickly because it’s not cross-linked or stabilised in a lab setting like your traditional cross-linked hyaluronic acid fillers. Some clinicians get 3 months in thicker areas with lesser animation or movement, but it’s variable and often optimistic.
The only “longer” effect comes from mild collagen stimulation from the platelets. It’s very subtle, and not structural. Patients expecting 6-month volume need counselling up front. It is considered as a pre-event volumiser and not a long term filler, especially for anyone looking for volume and is not ready to commit to or has had a reaction or adverse events with traditional hyaluronic acid fillers.
What “biofiller with PRF” means
You can combine PRF into the heated and then cooled plasma before it sets. This adds growth factors to the gel, giving both an instant lift and regenerative activity. The result is smoother texture and hydration, but it’s still very mild.
Under-eye use
This is where biofiller has most value. It’s smoother than HA filler, carries almost zero risk of Tyndall effect, and integrates naturally. However, like any filler, results are heavily practitioner dependent. Expect 4-6 weeks of visible improvement, sometimes up to 8. Results taper gradually.
What patients actually see
Immediately: mild swelling & puffiness
Week 2-3: finer texture, fresher look.
Week 6-8: most of the gel gone, but collagen improvement holds a little.
Side effects
Redness, swelling, bruising are common for 24-72 hours. Even the most experienced injectors cannot always avoid bruising.
Unevenness or lumping can happen if gel sets unevenly.
Vascular occlusion is rare but theoretically possible if injected into a vessel, so anatomical caution still applies.
Because it’s autologous (from self), allergic reaction is essentially zero.
Sculptra vs biofiller
Sculptra (poly-L-lactic acid) is a synthetic biostimulator. Once injected, the PLLA microparticles cause a mild, sterile inflammatory reaction that activates fibroblasts to lay down new collagen. It doesn’t add volume instantly; instead it builds collagen gradually over 2-3 months and lasts 18-24 months.
Biofiller gives immediate, gentle volume because of the gel, but disappears within weeks. It doesn’t generate a controlled inflammatory response like Sculptra.

Can it be dissolved
No. There’s no enzyme for albumin gel. If too much is placed or you dislike the shape, it can only be broken down mechanically or left to resorb in due course.
Downtime
Usually one to two days of mild swelling. Bruising up to a week. Most people resume work the next day.
PRF vs filler
PRF and biofiller build skin quality. Traditional fillers build structure and add volume.
They’re complementary. If a patient needs lift or projection, use filler.
If they need texture or regenerative improvement with temporary structural enhancements, use PRF biofiller. It is a good entry point into aesthetic injectables for those who are filler averse.
