Downtime

Mild Swelling

Marionette lines are the downward creases that run from the corners of the mouth toward the chin, often giving a tired or downturned expression.

They develop as midface support diminishes and the depressor anguli oris (DAO) muscles pull the mouth corners down.

At Dr Hans Clinics, treatment focuses on re-supporting the area to restore harmony and neutral expression.

We use Neauvia, Vivacy, Belotero, Juvederm and Revanesse, selected for appropriate lift capacity, softness, and reversibility.

All treatments are performed following a medical consultation and prescriber approval.

HOW MARIONETTE FILLER WORKS

With age, volume loss around the oral commissure and chin creates shadowing and downward folding.
By restoring volume in the pre-jowl sulcus and perioral area, hyaluronic-acid filler provides lift and structural continuity between the cheek, mouth corner, and chin.

Mechanism:

HA attracts water molecules, restoring dermal hydration and soft-tissue thickness.
Subdermal and deep bolus placement along the marionette groove re-supports the oral commissure.
Filler rheology (G′ and cohesivity) determines firmness and integration to maintain natural movement.
Onset: Immediate
Peak : 2 Months
Duration: Most fillers last 9 - 15 months, depending on the product and treatment area, as per the manufacturer’s guidelines. However, in practice, some filler volume may remain and continue to provide subtle effects for several years.

WHY PATIENTS CHOOSE DR HANS CLINICS

Doctor-led anatomical assessment to correct downward vectors without stiffness.
Conservative dosing focused on proportional restoration.
Use of micro-cannula to reduce bruising and vascular risk.
CE-certified products only, with full documentation of batch and lot numbers.

EVIDENCE & SAFETY

Clinical studies confirm hyaluronic-acid fillers are effective for lower-face rejuvenation when performed by qualified medical practitioners (Aesth Surg J 2020; J Clin Aesthet Dermatol 2021).
Temporary swelling or tenderness is expected for 24 - 48 hours.
Rare complications include oedema, Tyndall effect (blueish hue), or vascular compromise - mitigated by appropriate product choice and anatomical precision.
Serious complications such as vascular occlusion are exceedingly rare and mitigated by sterile technique, aspiration, and reversal protocols.
Contra-indications: active skin infection, pregnancy, unhealed wounds, or recent dental work.

TREATMENT PLAN & TECHNIQUE

Consultation: Assessment of facial symmetry, vector analysis, and muscle pull.
Technique:
  • Deep linear threads or micro-bolus placement in pre-jowl sulcus and marionette groove.
  • Superficial refinement to smooth shadow line.
  • Cannula used in most cases for improved control.
Anaesthesia: Topical or local if required.
Procedure time: 30 - 45 minutes including consultation.
Follow-up : 2 weeks to assess integration and symmetry.

FAQ's

What is PRF EZ Gel?

PRF EZ Gel is an injectable treatment made from your own blood, plasma component. A small blood sample is taken, processed, and prepared into a soft gel-like material by heating and then cooling the plasma that can be used to support skin quality, soften fine lines, and improve mild hollowing or crepiness in selected areas. The Plasma contains albumin a protein, that when heated and cooled turns into a gel.

PRF EZ Gel is commonly used for under-eye hollows, crepey under-eye skin, fine lines, mild facial volume loss, neck lines, décolletage, hands, and general skin quality improvement. It is best suited for subtle, natural-looking improvement rather than dramatic reshaping.

No. Dermal filler is usually made from hyaluronic acid and is designed to create volume, contour, and structure. PRF EZ Gel is made from your own blood and works more gently. It may provide soft tissue support, but it is not a direct replacement for filler.

Before booking PRF EZ Gel for my under eyes, I had a lot of questions and concerns. Dr Hans took time to go through all of them properly and did not rush the consultation. She explained the treatment in a balanced way and set realistic expectations. The procedure itself was more comfortable than I imagined and the result looks very natural. My face looks more rested rather than noticeably treated, which was important to me.

I had a great experience with Dr. Hans. I wanted to do Ez gel under my eyes. She listened to my concerns and took her time to explain why it wouldn’t be suitable treatment for me. Instead we agreed to tighten my skin with Tixel - I had 3 sessions, very happy with the results.

Absolutely the most knowledgeable and kindest doctor I’ve met. I’ve had injectables and PRF and soon tixel with her. She makes you feel so comfortable and confident. She will also say no and not just try to sell which is so important. She truly wants to improve the life of others which isn’t the case throughout the aesthetic industry (a lot of people try to exploit insecurities) and I used to work in it so I know! Thank you so much
