They aren’t.
They are different tools.
Different preparations.
Different objectives.

Over the years I’ve performed and assessed thousands of PRP, PRF, hair restoration, facial rejuvenation and regenerative procedures including fat grafting and liposuction. One observation keeps resurfacing. The treatments that generate the most excitement are not always the treatments that survive the test of time. Fat grafting, microfat and nanofat continue to appear in surgical and aesthetic practice because they are built around something simple: replacing or supporting tissue with the patient’s own tissue. That does not make them perfect. It does make them difficult to ignore when discussing regenerative facial rejuvenation.
The Problem With The Word “Regenerative”
Before discussing fat, we need to address the elephant in the room.
Regenerative has become one of the most abused words in aesthetic medicine.
Patients hear the word and understandably assume it means:

- Restoring youth
The reality is usually less dramatic.
Many treatments marketed as regenerative are attempting to improve skin quality, hydration, healing or collagen production.
Those outcomes can be valuable.
They simply are not the same thing as regenerating tissue.

It isn’t.

But they are not replacing tissue.
They are not restoring volume.
They are not transferring living cells.
Fat transfer is fundamentally different because the objective is not simply signalling.
The objective is replacement.
If volume has been lost, volume is being restored.
If tissue quality improves as a secondary benefit, that’s an additional advantage rather than the primary goal.
This distinction matters because many regenerative treatments are discussed as though they occupy the same category.
They don’t.

The names sound similar.
The objectives are not.
In the next section we’ll break down exactly what microfat, nanofat and Seffiller are, how they differ, where the evidence sits for each, and why choosing the wrong one for the wrong problem is one of the commonest mistakes in regenerative aesthetics.
Microfat, Nanofat and Seffiller: What Are You Actually Injecting?
One of the biggest mistakes I see is people discussing microfat, nanofat and Seffiller as though they’re different brand names for the same treatment.
They aren’t.
In fact, they often have completely different objectives.
The easiest way to understand them is to stop thinking about the harvesting process and start thinking about the goal.
Are we trying to restore volume?
Improve skin quality?
Improve scars?
Improve under-eye quality?
Support tissue repair?
Or a combination of several of those things?
Because each preparation was developed to solve a slightly different problem.
Microfat: When Volume Is The Main Goal
Microfat is probably the closest thing to traditional fat grafting.
The harvested fat is processed and refined, but the mature fat cells remain largely intact.
That’s important.
Because mature fat cells are responsible for volume.
If you’re treating:
- Hollow temples
- Midface volume loss
- Facial wasting
- Deep tear trough deformity
- Structural volume loss
microfat makes far more sense than nanofat.
Why?
Because nanofat contains very few viable mature fat cells.
Microfat still contains them.
In simple terms, microfat is designed to survive.
Nanofat is designed to signal.

Facial fat transfer has been performed for decades and has a substantial body of literature supporting its use for facial volume restoration. Long-term retention varies considerably between studies, techniques and anatomical sites, but successful graft survival can persist for many years. The variability is frustrating. The longevity can be impressive. Both statements are true.

The strongest evidence is probably in scar management and skin quality improvement rather than dramatic rejuvenation. The literature remains relatively small, but the rationale is more convincing than many patients realise because the treatment is derived from the patient’s own tissue rather than an external product. Published studies have reported improvements in scar appearance, skin quality and periorbital rejuvenation, although many studies remain small and further high-quality trials are still needed.
This is also where nanofat differs from most commercial exosome products.
You’re not injecting isolated messenger particles.
You’re injecting a complex mixture derived from living tissue.
Whether that translates into superior outcomes remains an area of ongoing investigation, but biologically the comparison is not straightforward.
So Where Does Seffiller Fit?
This is where the conversation becomes particularly interesting.
Because Seffiller isn’t really a new biological concept.
It’s a harvesting and processing system.
That distinction matters.
The marketing sometimes focuses on the device.
The important part is the tissue.
Seffiller was designed to harvest and process adipose tissue in a way that preserves as much of the regenerative component as possible while allowing the clinician to create preparations suitable for different indications.
Depending on processing, Seffiller can be used to create products that behave more like microfat or preparations that move closer towards nanofat-style applications.
What attracts many clinicians to systems such as Seffiller is not that they magically create regeneration.
It’s that they attempt to preserve what already exists within the tissue.
The reality is that adipose tissue is biologically active.
It contains a rich stromal vascular fraction, often abbreviated to SVF.
This includes:
- Stromal cells
- Pericytes
- Endothelial progenitor cells
- Immune cells
- Adipose-derived stromal cells
This is one reason fat has attracted so much attention within regenerative medicine.
You’re not injecting a single molecule.
You’re transferring an entire ecosystem.
The Problem With Calling Everything “Stem Cells”
This is another area where marketing often runs ahead of the evidence.
Patients frequently hear:
“Stem-cell facial.”
“Stem-cell fat transfer.”
“Stem-cell rejuvenation.”
In reality, most procedures are not harvesting purified stem cells.
They are transferring tissue that contains a mixture of cell populations, some of which possess stem-like properties.
That may sound like a technical distinction.
It’s actually an important one.
Because it changes expectations.

Probably the biggest lesson is that volume and skin quality are not the same thing.
A patient may be delighted with volume retention and notice little change in skin quality.
Another patient may notice substantial improvement in skin quality despite relatively modest volume retention.
The treatment may have technically succeeded in both cases.
The patient’s definition of success may be completely different.
This is why consultation matters so much.
One of the easiest ways for patients to become disappointed is to pursue a treatment before defining the outcome they actually want.
Microfat, nanofat and Seffiller are not competitors.
They’re tools.
The important question is not which one is best.
The important question is what problem you’re trying to solve.
Bioengineered Exosomes
This is where the field starts becoming genuinely fascinating.
Researchers can modify cells to produce vesicles carrying specific cargo.
In theory, this could allow targeted delivery of highly specific messages.
In practice, much of this remains within advanced research rather than routine aesthetic medicine.
When people talk about the future of exosomes, this is often what they mean.
The challenge is that the future and the present are not the same thing.
Many conference presentations discuss where the field may be heading.
Patients deserve to know where the field currently is.
Recombinant And Synthetic Systems
Some products contain recombinant proteins, peptides or synthetic delivery systems designed to reproduce selected aspects of extracellular vesicle function.
These products may eventually prove useful.
The important point is that they should not automatically be treated as interchangeable with naturally derived extracellular vesicles.
Again, the word exosome often creates the illusion that all products belong to the same family.
The reality is considerably messier.
Why This Matters To Patients
One of the biggest mistakes in aesthetics is assuming that treatments sharing a label are therefore comparable.

Patients rarely see those differences.
They see the same buzzword.
This is one reason I am cautious about broad claims.
It is also one reason I think patients deserve more transparency than they currently receive.
Because before asking whether exosomes are better than polynucleotides, we first need to establish which exosomes we are talking about.
Only then can we have a meaningful conversation.
What Are Polynucleotides? Why They Are Not Exosomes, Not Fillers, And Not Quite What Most Patients Think They Are
If exosomes are currently the most talked-about treatment in regenerative aesthetics, polynucleotides are probably the most misunderstood.
One of the reasons for this confusion is that polynucleotides don’t fit neatly into an existing category.
They’re not fillers.
They’re not skin boosters in the traditional sense.
They’re not exosomes.
They’re not growth factors.
And they’re certainly not tiny packets of salmon swimming around under the skin, despite what some social media comments might have you believe.
Yet somewhere between the marketing, conference presentations and Instagram reels, many patients have been left with the impression that polynucleotides are capable of rebuilding skin, reversing ageing and somehow persuading tired tissue to behave like it did twenty years ago.
The reality is less dramatic.
And, in my opinion, more interesting.
Where Do Polynucleotides Come From?
Most commercially available polynucleotide products are derived from highly purified DNA fragments obtained from salmonid species, typically salmon or trout.
That immediately raises eyebrows.
Patients often ask:
“So you’re injecting fish DNA into my face?”
Not exactly.
The important word is purified.
What remains after manufacturing is not a piece of salmon tissue. It is a highly processed and purified collection of DNA fragments designed for medical use.
The rationale behind using salmon-derived material is not random.
Salmon DNA shares structural similarities with human DNA and has historically been used in various medical applications because of its biocompatibility.
The manufacturing process is intended to remove proteins and other components that might trigger unwanted immune responses.
The final product is very different from the source material it originated from.
Polynucleotides And PDRN Are Not The Same Thing
This is another area where confusion is common.
People often use PDRN and polynucleotides
interchangeably.
Strictly speaking, they are related but not identical.
PDRN stands for polydeoxyribonucleotide.
It consists of smaller DNA fragments.
Polynucleotides generally refer to longer-chain DNA fragments.
Both have been studied in wound healing and tissue repair.
Both are often discussed under the same umbrella.
However, they are not necessarily identical products and should not automatically be treated as such.
This becomes important when comparing studies because one paper may be investigating PDRN while another is investigating a polynucleotide preparation.
Unfortunately, those differences are often lost once the treatment reaches social media.

This is probably the section that will annoy the most people.
Not because it’s controversial.
Because it forces us to compare treatments that are usually marketed in isolation.
Every company wants you to look at their product.
Very few want you comparing it with alternatives.
Patients deserve that comparison.
Because if you’re considering nanofat treatment London, Seffiller treatment London, PRP, polynucleotides or exosomes, the obvious question is:
Which actually has the strongest evidence?
The answer depends on what outcome we’re measuring.
That distinction matters.
A treatment may have excellent evidence for wound healing and weak evidence for facial rejuvenation.
Another may have encouraging laboratory data and very little human data.
Those are not equivalent.
Tier 1: Facial Fat Grafting For Volume Restoration
If we’re talking about restoring lost facial volume, fat grafting remains one of the strongest regenerative procedures available.
Not because the evidence is perfect.
Not because retention is predictable.
But because we have decades of experience, long-term follow-up studies and a large surgical literature.
Unlike fillers, successful fat grafts can potentially survive for many years.
The trade-off is variability.
Some fat survives.
Some doesn’t.
No clinician can honestly guarantee retention percentages.
Anyone who does should make you nervous.
For volume restoration, however, fat transfer sits comfortably at the top of this list.
Tier 2: Microfat
Microfat sits very close to traditional fat grafting.
The primary objective remains tissue replacement.
The evidence base is therefore heavily supported by the broader fat grafting literature.
This is one reason many facial plastic surgeons and reconstructive surgeons continue to use microfat despite the emergence of newer regenerative products.
The concept is simple.
Lost tissue is replaced with living tissue.
Not every treatment can say that.
Tier 3: Nanofat
Nanofat is where things become more complicated.
The evidence is smaller.
The outcomes are harder to measure.
And the objectives are different.
We’re no longer talking about volume.
We’re talking about:
- Skin quality
- Scar improvement
- Texture
- Periorbital rejuvenation
- Tissue quality
The literature is encouraging.
Several studies report improvements in acne scars, skin quality and under-eye appearance.
The problem is that most studies remain relatively small.
This does not mean nanofat doesn’t work.
It means the confidence should match the evidence.
At present, nanofat probably occupies one of the more promising positions in regenerative aesthetics, but it still requires larger, independent studies.
Tier 4: PRP
This is where people become surprised.
PRP is often dismissed because it is no longer fashionable.
That is unfortunate.
Because PRP has something many newer treatments lack.
Human clinical data.
Is the evidence perfect?
No.
The preparation methods vary enormously.
The protocols vary enormously.
The quality of studies varies enormously.
Yet there is still more human clinical evidence for PRP than many commercial exosome products currently marketed in aesthetics.
That reality is sometimes inconvenient.
Especially for people selling exosomes.
Tier 5: Polynucleotides
Polynucleotides probably sit somewhere around the same neighbourhood as PRP for aesthetic indications.
Potentially slightly lower or slightly higher depending on the indication being discussed.
The strongest support exists for:
- Skin quality
- Hydration
- Periorbital rejuvenation
- Recovery support
The biggest limitation remains study quality and study size.
Promising?
Yes.
Settled science?
No.
Tier 6: Human MSC-Derived Exosomes
This is where expectations and evidence begin to diverge.
The biological rationale is attractive.
The laboratory data is interesting.
The animal data is encouraging.
Human evidence remains relatively limited.
That doesn’t mean exosomes are ineffective.
It means we should be cautious about making claims that exceed the available evidence.
Tier 7: Plant, Marine, Biomimetic And Engineered Exosomes
This category sits lower not because these products are necessarily inferior.
Simply because the evidence base is currently smaller.
Many products have fascinating concepts behind them.
Concepts are not outcomes.
Patients pay for outcomes.
And outcomes require data.
What Is The Closest Thing To True Regenerative Aesthetics?
If somebody forced me to answer that question today, my answer would not be exosomes.
It would not be polynucleotides.
It would not be PRP.
It would be fat.
Not because fat is perfect.
Because fat is alive.
And when living tissue survives, it continues behaving like living tissue.
That remains one of the most compelling arguments in regenerative aesthetics today.
Seffiller is not nanofat.
And Seffiller is not pure SVF either.
This distinction is important because many trainers and distributors blur the lines.
What is Seffiller actually?
Seffiller is primarily a:
Closed-system fat harvesting and processing platform.
The device itself doesn’t create a new tissue type.
It harvests and processes adipose tissue.
What you end up injecting depends on how you process the harvested tissue afterwards.
So Seffiller can be used to produce something closer to:
- Microfat
- Millifat
- Structural fat graft
- Mechanically emulsified fat
- Nanofat-like preparations
depending on technique.
Is Seffiller SVF?
Not really.
True SVF (stromal vascular fraction) is usually obtained through:
- Enzymatic digestion (collagenase)
- Centrifugation
- Cell isolation
That produces a concentrated cellular fraction.
Most Seffiller procedures do not produce true isolated SVF.
What they produce is:
Fat tissue that still contains SVF components.
Very different.
This is also why Seffiller is easier to perform legally in many jurisdictions than enzymatic SVF isolation.
Is Seffiller nanofat?
Again, not automatically.
Nanofat is a specific product first described by Tonnard et al.
Nanofat requires:
- Mechanical emulsification
- Repeated passage between syringes
- Progressive filtration
The resulting preparation contains very few intact mature adipocytes.
If you harvest using Seffiller and then perform nanofat processing, you can create nanofat.
But the Seffiller harvest itself is not nanofat.
Think of it like this:
Seffiller = kitchen
Microfat, nanofat, structural fat = meals
The kitchen doesn’t determine the meal.
The preparation does.
The Downsides Nobody Mentions About Fat-Based Regenerative Treatments
At this point, it would be very easy to make fat transfer sound like the perfect regenerative treatment.
It isn’t.
In fact, one of the reasons I remain cautious whenever people describe anything as a miracle treatment is because every procedure has trade-offs.
Fat transfer is no exception.
The discussion becomes far more useful once we stop asking:
“What are the benefits?”
and start asking:
“What are the limitations?”
Because those limitations often determine whether somebody becomes delighted with the result or disappointed by it.
The Biggest Problem: Fat Doesn’t Read Textbooks
One of the most frustrating aspects of fat grafting is that biology refuses to behave predictably.
You can perform the same procedure.
Use the same technique.
Treat the same area.
And still obtain different outcomes between patients.
Some patients retain volume exceptionally well.
Others experience substantial resorption.
This is not necessarily a sign that something went wrong.
It is simply the reality of working with living tissue.
A syringe of filler behaves relatively predictably.
Living fat behaves like living fat.
And living tissue has a habit of making its own decisions.
Volume Retention Is Never Guaranteed
This is probably the most important conversation I have before any fat-based procedure.
Nobody can honestly guarantee how much fat will survive.
Anyone quoting exact percentages should be approached with caution.
The literature reports a wide range of retention rates because outcomes depend on:
- Harvest technique
- Processing
- Injection technique
- Recipient site
- Patient factors
- Smoking
- Weight fluctuations
- Blood supply
- Inflammation
One patient may retain a large proportion of transferred volume.
Another may require additional sessions.
Neither outcome is unusual.
The Results Take Longer
Modern aesthetics has created an expectation that every treatment should provide immediate gratification.
Fat transfer doesn’t always cooperate.
Some changes occur quickly.
Others take months to declare themselves.
Swelling settles.
Tissues remodel.
Some fat survives.
Some doesn’t.
Patients seeking instant transformation often struggle with this timeline.
The irony is that many of the treatments marketed as “natural” require considerably more patience than fillers.
It’s More Invasive Than Most Injectable Treatments
This point is often overlooked.
To obtain fat, we first need to harvest fat.
That means:
- Local anaesthetic
- Small incisions
- Harvesting
- Processing
- Reinjection
Compared with PRP, polynucleotides or exosomes, the procedure is significantly more involved.
That doesn’t make it worse.
But it does mean patients should understand that this is not simply another injectable treatment.
Not Everybody Has Suitable Donor Fat
Another reality rarely discussed in marketing material.
Some patients simply do not have ideal donor sites.
Very lean patients can be more challenging.
This does not automatically rule out treatment.
It simply means planning becomes more important.
Lumps, Irregularities And Overcorrection Can Occur
Although uncommon in experienced hands, fat transfer is not free from complications.
Potential issues include:
- Uneven volume retention
- Irregularities
- Fat necrosis

- Calcification
- Overcorrection
- Under-correction

“How much regeneration occurred?”
The honest answer is that nobody can quantify that precisely.
Volume can be measured.
Photographs can be compared.
Patient satisfaction can be assessed.
The broader regenerative claims become more difficult to measure objectively.
This is not unique to fat.
It applies equally to PRP, exosomes and polynucleotides.
The challenge is that the word regenerative often sounds more measurable than it actually is.
Why Some Patients Become Disappointed
The reason is surprisingly simple.
Patients often expect fat transfer to do everything.
Restore volume.
Improve skin quality.
Tighten skin.
Improve scars.
Reverse ageing.
Sometimes several of those outcomes improve simultaneously.
Sometimes they don’t.
The problem begins when one treatment is expected to solve every problem.
The best outcomes occur when expectations are aligned with reality.
A patient seeking volume restoration may be thrilled.
A patient expecting a non-surgical facelift may be disappointed.
The procedure may not have changed.
The expectation did.
Why Fat Transfer Is Still Worth Discussing
After reading all of this, you might wonder why clinicians continue to use fat-based treatments at all.
The answer is straightforward.
Despite the limitations, fat remains one of the few treatments capable of genuinely replacing lost tissue.

One that deserves more attention than it often receives in today’s race towards newer and more marketable treatments.
What I’ve Learnt After Treating Patients Seeking “Regenerative” Treatments
If there’s one thing I’ve learnt after years of treating patients interested in PRP, fat transfer, nanofat, polynucleotides, skin boosters and every other treatment that has passed through the regenerative medicine spotlight, it’s this:
Most patients don’t actually want regeneration.
They want a problem solved.
The distinction sounds trivial.
It isn’t.
Nobody wakes up one morning and thinks:
“I’d really like more extracellular vesicles.”
Nobody looks in the mirror and says:
“My stromal vascular fraction seems a bit low.”
Patients don’t care about acronyms.
They care about outcomes.
They care about looking less tired.
Less hollow.
Less aged.
Less weathered.
Less self-conscious.
And that is where many regenerative treatments begin to lose their way.
Because somewhere between the laboratory, the conference stage and the clinic consultation, the focus often shifts from solving problems to selling products.
The Newest Treatment Is Rarely The Most Important
One pattern I’ve noticed repeatedly is that patients often arrive convinced they already know what they need.
Last year it was exosomes.
This year it may be polynucleotides.
Next year it will probably be something else.
The names change.
The behaviour doesn’t.
Aesthetic medicine has always had trends.

A treatment can go from an early conference presentation to a social media sensation within months.
The result is that patients frequently ask for treatments before they understand what problem those treatments are supposed to solve.
The irony is that some of the best outcomes I’ve seen have come from treatments that are no longer considered fashionable.
Fat transfer.
PRP.
Conservative filler.
Good skincare.
Sun protection.
Lifestyle changes.
None of those generate the same excitement as a new product launch.
Yet they continue to produce results.
The Industry Often Rewards Novelty More Than Evidence
This isn’t unique to aesthetics.
It happens in every industry.
New sells.
Old doesn’t.
It’s far easier to market a breakthrough than it is to market consistency.
The problem is that evidence takes time.
Good evidence takes even longer.
A treatment can become popular years before we properly understand its strengths and weaknesses.
I’ve seen this happen repeatedly.
A treatment arrives.
Conference speakers become enthusiastic.
Manufacturers become enthusiastic.
Clinics become enthusiastic.
Patients become enthusiastic.
Then reality slowly catches up.
Sometimes the treatment survives that process.
Sometimes it doesn’t.
Most end up somewhere in the middle.
Useful.
But not revolutionary.
Why Patients Become Disappointed
Contrary to what many people think, disappointment is rarely caused by a bad treatment.
More often it’s caused by a mismatch between expectation and reality.
A patient hears:
“Regenerative.”
What they imagine is:
“Restorative.”
Those are not always the same thing.
Improving tissue quality is not the same as restoring youthful anatomy.
Improving hydration is not the same as reversing ageing.
Improving skin quality is not the same as replacing lost volume.
When those distinctions aren’t explained properly, disappointment becomes almost inevitable.
The Treatments That Tend To Age Well
One observation I’ve made over time is that treatments with realistic claims tend to survive.
Treatments with exaggerated claims tend to struggle.

Why We Sometimes Recommend Doing Less
This is probably the least popular thing a clinic can say.
Sometimes the right answer is to do less.

There are patients who genuinely benefit from nanofat.

In reality, the success of microfat, nanofat and Seffiller depends far more on patient selection than the device, the syringe or the marketing brochure.

The first question should never be:
“What treatment do I need?”
The first question should be:
“What am I trying to improve?”

The Ideal Microfat Patient
Microfat is primarily a volume treatment.
Its greatest strength is restoring tissue that has physically disappeared over time.
The patients who tend to benefit most are those with:
- Hollow temples
- Midface volume loss
- Deep tear trough deformity
- Facial wasting
- Age-related fat loss
- Significant structural hollowing
These patients often look tired even when they feel perfectly well.
The problem is not necessarily skin quality.
The problem is that tissue has been lost.
Replacing lost tissue usually makes more sense than attempting to signal the body into behaving differently.
This is one reason microfat often competes more directly with fillers than with exosomes or polynucleotides.
The Ideal Nanofat Patient
Nanofat is almost the opposite.
The patient who benefits from nanofat is usually not looking for volume.
They’re looking for tissue quality.

And different causes require different solutions.

This is probably the section most patients care about.
Not because they want a lesson in regenerative medicine.
Because they want to know where to spend their money.
And honestly, I don’t blame them.
Most regenerative treatments are not cheap.
When somebody is considering PRP, PRF EZ Gel, nanofat, polynucleotides or exosomes, they’re often hearing very similar claims:
- Tissue repair
- Collagen stimulation
- Regeneration
- Skin rejuvenation
- Anti-ageing
- Natural results
The language sounds remarkably similar.
The treatments are not.

One of the biggest mistakes in aesthetics is comparing treatments that aren’t actually competing.

If somebody has significant volume loss beneath the eyes, comparing exosomes with nanofat is like comparing a paintbrush with a bricklayer.

Nanofat sits high on my list because it is derived from the patient’s own tissue and contains a complex mixture of cells, extracellular matrix fragments, growth factors and signalling molecules.

The downside is that procedures are more invasive and the evidence remains smaller than many people realise.
Why Polynucleotides Rank Highly
Polynucleotides have become popular because the treatment generally does what it claims to do reasonably well.
The problem is not usually effectiveness.
The problem is marketing.
Some companies have positioned polynucleotides as though they are capable of dramatic rejuvenation.
The evidence doesn’t support that.
Improved skin quality?
Reasonable.
Improved hydration?
Reasonable.
Improved texture?
Reasonable.
A non-surgical facelift?
No.
Where PRF EZ Gel Fits
PRF EZ Gel is one of the most interesting treatments currently available because it sits somewhere between a filler and a regenerative treatment.
Unlike PRP, which is largely liquid, PRF EZ Gel creates a fibrin scaffold.
That scaffold can:
- Hold volume temporarily
- Slow growth factor release
- Provide structural support
The attraction is obvious.
The challenge is longevity.
The reality is that PRF EZ Gel does not survive like fat.
It does not persist like a long-lasting filler.
Patients considering PRF EZ Gel should understand that it occupies a middle ground.

It may provide modest volume.
But it should not be viewed as a replacement for fat transfer when significant volume restoration is required.
Why PRP Still Deserves More Respect
PRP has become oddly unfashionable.
Partly because it isn’t new.
Partly because it isn’t exciting.
Partly because nobody can patent your own blood.
Yet when you look at the literature, PRP still has something many regenerative treatments lack.
Human clinical evidence.
Not perfect evidence.
Not standardised evidence.
But human evidence nonetheless.
The biggest weakness of PRP is variability.
Two clinics can both advertise PRP and deliver completely different products.
Different centrifuges.
Different platelet concentrations.
Different activation methods.
Different protocols.
That makes interpretation difficult.
Where Exosomes Sit Today
This section will probably upset somebody.
Exosomes are fascinating.
The research is interesting.
The future may be very bright.
But if we are talking about aesthetic outcomes today rather than future possibilities, I still believe many commercial exosome products are running ahead of their evidence.
That doesn’t mean they don’t work.
It means the certainty often exceeds the data.
One of the challenges with exosomes is that patients frequently assume all exosomes are the same.
They’re not.
Human-derived exosomes, plant-derived exosomes, biomimetic exosomes and engineered exosomes should not automatically be discussed as though they are equivalent products.
Unfortunately, they often are.
What Would I Choose For Myself?
If I developed volume loss tomorrow, I would not choose exosomes.
I would not choose polynucleotides.
I would not choose PRP.
I would look at fat transfer first.
If my concern was skin quality, scarring or crepey under-eye skin, the conversation becomes more nuanced.
Nanofat would move much higher on the list.
Polynucleotides would become reasonable.

Not because exosomes are uninteresting.
Not because exosomes don’t work.
But because living tissue remains more convincing to me than isolated products attempting to replicate what living tissue already does naturally.

The best choice depends entirely on the problem being treated.
Is Seffiller The Same As Nanofat?
No.
This is one of the most common misunderstandings.
Seffiller is a harvesting and processing system.
Nanofat is a specific tissue preparation.
A clinician can harvest tissue using Seffiller and then process it into nanofat.
But Seffiller itself is not nanofat.
Think of Seffiller as the kitchen.
Nanofat is one of the meals you can prepare inside it.
Is Seffiller The Same As Stem Cell Therapy?
Not really.
The phrase stem cell therapy is often used very loosely in aesthetics.

Patients become enthusiastic.
Suddenly every concern is being solved with the same treatment.
The reality is that no treatment deserves that level of confidence.
The moment a treatment starts being recommended for everything, I usually become more cautious rather than less.
Because medicine rarely works that way.
The best treatments tend to have specific indications.
Specific strengths.
Specific weaknesses.
And specific limitations.
We Focus On The Diagnosis First
Most patients don’t arrive with a treatment problem.
They arrive with a tissue problem.
That may sound obvious, but it changes the entire conversation.
A patient with volume loss needs a different discussion from a patient with poor skin quality.
A patient with acne scarring needs a different discussion from somebody with facial hollowing.
A patient with crepey under-eye skin needs a different discussion from somebody with visible veins.
Those distinctions matter.
Because once the diagnosis changes, the treatment often changes too.
This is one reason we spend far more time discussing the cause of a concern than discussing the latest product.
We Are Comfortable Saying “I Don’t Think You Need That”
This is probably one of the least profitable things a clinic can do.
It’s also one of the most important.
Not every patient needs nanofat.
Not every patient needs Seffiller.
Not every patient needs PRP.
Not every patient needs exosomes.
And certainly not every patient needs a treatment simply because it is currently trending.
Sometimes the most valuable thing a clinician can say is:
“I don’t think this is the right treatment for you.”
Those conversations are often remembered far longer than the treatments themselves.
We Are More Interested In Long-Term Outcomes Than Immediate Excitement
Aesthetic medicine has become increasingly focused on novelty.
The problem is that novelty and evidence rarely arrive at the same time.
Good evidence takes years.
Sometimes decades.
This is one reason treatments such as fat transfer continue to interest me.
Not because they’re new.
Because they aren’t.
Quite the opposite.
They’ve survived long enough for us to understand many of their strengths and weaknesses.
The same principle applies to every treatment discussed in this article.
The goal is not to chase the newest option.
The goal is to find the most sensible option.
The Most Honest Answer Is Sometimes “We Don’t Know Yet”
This may be the least exciting sentence in medicine.
It’s also one of the most important.
There are aspects of exosome therapy we still don’t fully understand.
There are aspects of polynucleotides we still don’t fully understand.
There are aspects of nanofat and adipose-derived therapies we still don’t fully understand.
That uncertainty shouldn’t frighten patients.
It should reassure them.
Because uncertainty acknowledged is far safer than certainty invented.
Whenever a clinician sounds absolutely certain about a rapidly evolving area of medicine, it is worth asking a few more questions.

It was written to help you think about treatments differently.
Final Thoughts: Why Fat Has Outlived Almost Every Trend In Aesthetic Medicine
If you’ve spent enough time around aesthetic medicine, you start noticing a pattern.
Every few years, a new treatment arrives promising to change everything.
Before that it was stem cells.
Then growth factors.
Then PRP.
Then exosomes.
Then polynucleotides.
Tomorrow it will be something else.
The names change.
The marketing changes.
The conference slides change.
The basic biology rarely does.
One of the reasons fat transfer has survived decade after decade is because it solves a problem that actually exists.
Ageing causes volume loss.
Fat restores volume.
The logic is difficult to argue with.
That doesn’t mean fat transfer is perfect.
It isn’t.
Results vary.
Retention varies.
Additional procedures are sometimes needed.
But unlike many treatments marketed under the regenerative umbrella, fat is not trying to imitate tissue.
It is tissue.
That distinction matters.
After seeing thousands of patients over the years, I’ve become less interested in treatments that sound impressive and more interested in treatments that consistently solve problems.
Sometimes that means PRP.
Sometimes that means PRF EZ Gel.
Sometimes that means polynucleotides.
Sometimes that means nanofat.
And sometimes it means doing absolutely nothing.
The difficult part is not finding a treatment.
The difficult part is finding the right treatment.
Patients often ask me whether nanofat treatment, Seffiller treatment, PRF EZ Gel, exosomes or polynucleotides are “better”.
The honest answer is that they’re often being asked to do different jobs.
A patient with hollow temples requires a different solution from a patient with acne scars.
A patient with crepey under-eye skin requires a different solution from a patient with volume loss.
The diagnosis comes first.
The syringe comes second.
That philosophy influences every consultation at Dr Hans Clinics.
We don’t start with the latest trend.
We start with the problem.
If that sounds obvious, you’d be surprised how often it’s forgotten.

Be wary of certainty.
Be wary of miracle claims.

Because the goal isn’t to sell regeneration.
