This is why I do not position Tixel as “weaker CO2”.

That is lazy.

I see Tixel as a different kind of resurfacing tool. Less theatrical. More modular. Easier to combine. Often better suited to real humans with jobs, faces and lives.

Why Tixel can make more sense than CO2 for many patients

CO2 laser is powerful, but power is not always the goal.

If you have deep acne scarring or severe photodamage, CO2 may be the better treatment. No drama. Credit where credit is due.

But many patients do not need the nuclear option.

They have:

Crepey under-eye skin
Fine lines
Rough texture
Dullness
Early laxity
Mild acne scarring
Enlarged pores
Post-inflammatory unevenness
Thin eyelid skin
Skin that looks tired rather than destroyed

For these patients, Tixel can be a very logical option because it allows controlled skin injury without the same level of intensity as aggressive ablative laser.

That matters especially in patients who:

Cannot take one to two weeks of obvious downtime
Are prone to pigmentation
Want eyelid or under-eye treatment
Prefer staged improvement
Want combination treatment with PDRN, polynucleotides, PRP or recovery-focused actives
Need a treatment that can be adjusted over a course

This is where Tixel has a quiet advantage.

It does not have to shout. It just has to make sense.

The eyelid and under-eye advantage

This is one of the biggest reasons I like Tixel.

The eye area is delicate. Thin skin, poor tolerance for swelling, high visibility, and very little room for error.

CO2 laser can be used around the eyes by experienced clinicians, but it is not a casual choice. It needs proper expertise, eye protection (intraoccular eye shields (scary as hell), careful parameters and honest downtime counselling.

Tixel is commonly discussed for eyelid laxity, under-eye crepiness and periorbital rejuvenation because it can be used close to delicate areas with controlled settings. Reviews and clinic studies describe Tixel use for eyelid rejuvenation and periorbital skin improvement, though patient selection and settings remain critical. 

This does not mean Tixel is risk-free.

Nothing worth doing to skin is risk-free.

But for crepey eyelids and under-eye skin quality, Tixel often feels like the more sensible first conversation than aggressive CO2.

Less “burn the village to rebuild it”.

More “controlled renovation with planning permission”.

Tixel for darker skin types

This is where we need to be careful and not behave like marketing goblins.

Darker skin types can be treated with resurfacing devices, but the risk of post-inflammatory hyperpigmentation is higher with more aggressive heat and injury.

CO2 laser can absolutely be used in darker skin by experienced hands, but it needs caution, preparation and conservative parameters.

Tixel is often attractive in darker Fitzpatrick types because it avoids laser energy and can be adjusted for controlled thermal injury. That does not mean it is automatically safe for everyone. It still creates heat and inflammation. It still needs proper skin prep, aftercare and sensible settings.

In Fitzpatrick IV to VI skin, I would rather under-treat and build than over-treat and spend months managing pigmentation.

A face is not a proving ground for bravery.

Tixel and combination treatments

This is another reason I like Tixel.

Tixel can be used not just for resurfacing, but also as part of a delivery-style protocol.

When the skin has controlled microchannels or controlled thermal injury, selected topical actives may be used as part of a post-treatment plan. The 2024 review discusses Tixel’s role in drug delivery and aesthetic dermatology, including its ability to create channels that may support delivery applications. 

In clinic, this is where treatments like PDRN, PRP, calming serums or recovery-based actives may be considered depending on the patient.

Not because we want to throw everything at the face.

More is not always better. Sometimes more is just inflammation with an invoice.

The question is:

What does the skin need after controlled injury?

Barrier support?
Hydration?
Repair signalling?
Pigment control?
Inflammation reduction?
Collagen stimulation?

That is why Tixel fits a proper skin plan. It can be adjusted, staged and combined intelligently.

CO2 can also be combined with protocols, but the recovery burden is often higher, so the treatment planning is different.

Acne scars: is Tixel better than CO2?

For deep acne scars, no, not automatically.

CO2 laser is often stronger for more significant resurfacing and deeper textural work. If someone has deep boxcar scars, tethered rolling scars or ice-pick scars, they may need a combination plan involving subcision, TCA CROSS, microneedling RF, fractional laser, Tixel, PRP or other methods.

Tixel can help with texture and milder to moderate scarring, but it should not be sold as a one-treatment scar eraser.

That is how patients get disappointed and clinics start using the phrase “trust the process” a little too often.

For acne scars, I see Tixel as part of the toolbox.

Sometimes it is the main tool.
Sometimes it is a supporting tool.
Sometimes CO2 makes more sense.
Sometimes neither is the first step.

The scar type decides. Not the device brochure.

Downtime: the part patients actually care about

Let’s be honest.

Most people do not ask about collagen remodelling first.

They ask:

“How bad will I look?”
“How long before I can go out?”
“Can I work?”
“Will I peel?”
“Will I look insane on Zoom?”

CO2 laser usually has more downtime, especially when ablative settings are used. Redness, swelling, crusting, oozing, peeling and prolonged pinkness can happen depending on intensity.

Tixel downtime depends heavily on settings. Lighter protocols may give redness and roughness for a few days. Stronger protocols may create visible microcrusting, dryness and peeling.

So the honest answer is:

CO2 often gives more punch, but more punishment.

Tixel often gives less punishment, but may need a course.

And for many modern patients, that trade-off is not a weakness. It is the entire reason they choose it.

Is Tixel safer than CO2?

I would not use the word “safe” casually.

Both treatments create controlled injury. Both need trained hands. Both can cause burns, pigment changes, scarring, infection, prolonged redness or poor outcomes if used badly.

The difference is that Tixel usually allows a more controlled, staged approach with less aggressive downtime in many patients.

That can make it a better fit for:

Delicate areas
Eyelid skin
Busy patients
Darker skin types
Patients who prefer gradual improvement
Patients who want combination skin-quality protocols
Patients who are not ready for aggressive laser

But if someone needs significant resurfacing, CO2 may still be more appropriate.

This is not about making Tixel win every argument.

It is about making the right treatment win the right case.

My honest clinical view

I like CO2 laser.

But I respect it the way you respect a very sharp knife. Useful, powerful, not something to wave around because someone asked for “glow”.

I like Tixel because it fills a practical gap.

It gives us a non-laser fractional resurfacing option that can be used for skin quality, texture, crepiness, early laxity and eyelid skin with a more manageable recovery profile in many patients.

It is especially useful when the goal is not to obliterate the skin, but to coach it into repairing better.

That is why Tixel can make more sense than CO2 for many of my patients.

Not all.

Many.

That distinction matters.

CO2 is the heavyweight.

Tixel is the clever middleweight.

And in aesthetics, the heavyweight is not always the smarter choice. Sometimes it is just the one that leaves more evidence behind.

Final thoughts

If you are choosing between Tixel and CO2 laser, do not start with the device.

Start with the problem.

If you have deep wrinkles, severe sun damage or significant acne scarring, CO2 may be the stronger option.

If you have fine lines, crepey skin, eyelid laxity, mild to moderate texture, darker skin concerns, limited downtime or want a staged skin-quality plan, Tixel may be the more logical choice.

At Dr Hans Clinics, I do not choose Tixel because it is trendy.

I choose it when the biology, skin type, area and patient lifestyle make sense.

The best resurfacing treatment is not the most aggressive one.

It is the one your skin can recover from, benefit from and not punish you for.

Very unsexy. Very true.

Evidence and expectations

Tixel has published clinical and review evidence supporting its use as a thermo-mechanical fractional treatment for skin rejuvenation, acne scarring, eyelid skin and delivery-style applications. CO2 laser remains a well-established resurfacing option, especially for more intensive correction. Tixel may offer a more manageable downtime and risk profile for selected patients, but it is not a universal replacement for CO2 laser. Results vary depending on skin type, indication, settings, number of sessions and aftercare.
