How Microneedling Works (The Science)

Microneedling - also called collagen induction therapy - uses fine sterile needles to create controlled micro-injuries in the skin. This triggers a three-phase healing cascade:
1. Inflammatory Phase - platelets release PDGF, VEGF, TGF-β → signals fibroblasts and immune cells.

What is microneedling and how does it work?

Microneedling, or Collagen Induction Therapy, is a minimally invasive medical treatment. It uses ultra-fine, sterile needles to create controlled micro-channels in the skin. This process triggers the body’s natural healing response, which includes three phases: inflammatory, proliferative, and remodeling. It stimulates fibroblasts to produce new collagen and elastin. The outcome is structural repair that greatly improves acne scars, fine lines, large pores, and overall skin texture.

Comfort is a necessity. Before your treatment, a very effective topical numbing cream is applied to the target areas for about 20 minutes. Most patients describe the sensation during the procedure not as painful, but as a light scratching, vibrating or mild sandpaper-like feeling across the skin.

Downtime is minimal than with invasive laser resurfacing. Immediately after the procedure, your skin will feel tight and red, as if you had a mild sunburn. The inflammatory redness usually goes away about 90% within the first 24 hours and is a light pink color before disappearing completely in 2 to 3 days. As the skin renews itself, some very light flaking or dryness may occur.

Many patients notice glowing “glow” within 1 to 2 weeks but structural changes take time.

  1. For overall skin rejuvenation and texture enhancement: A course of 3 sessions is usually recommended as a baseline.
  2. For deep acne scars, stretch marks or advanced signs of aging, 3 to 6 sessions are recommended.
  3. Sessions are spaced 4 to 6 weeks apart to let your skin’s collagen cycle to fully mature.

To protect your freshly opened micro-channels and ensure optimal healing, you should follow these strict guidelines for the first 48 hours:

  • Avoid applying any makeup for at least 24 hours.

  • Do not use “active” skincare ingredients such as Retinols, Vitamin C, AHA/BHA chemical exfoliants, or glycolic acids for 3 to 5 days.

  • Avoid vigorous exercise, saunas, steam rooms, and excessive sweating for 48 hours.

  • Protect your skin daily with a broad-spectrum, zinc-based physical sunscreen (SPF 30 or higher) and keep it hydrated with gentle hyaluronic acid.

Yes. Unlike certain resurfacing lasers that carry a risk of hyperpigmentation in darker skin tones, medical microneedling is mechanically driven and entirely safe for all Fitzpatrick skin types. Because it does not use thermal heat, it delivers natural, reliable results without the risk of post-inflammatory hyperpigmentation.

While it is an incredibly versatile treatment, microneedling is not suitable for individuals who:

  • Have active acne breakouts, open wounds, skin infections, or active herpes simplex (cold sores) in the treatment area.

  • Are pregnant or breastfeeding.

  • Have a history of keloid or hypertrophic scarring.

  • Have undergone radiation therapy or taken Isotretinoin (Accutane) within the last 6 to 12 months.
